Enalapril was one of the first angiotensin-converting enzyme (ACE) inhibitors marketed for cardiovascular disease, with FDA approval in 1985. It is an oral prodrug that is converted to the active metabolite enalaprilat after absorption, and it acts by suppressing the formation of angiotensin II. The result is vasodilation, reduced aldosterone secretion, and decreased preload and afterload, which collectively lower blood pressure and improve cardiac performance in susceptible patients.
As a member of the ACE inhibitor class, enalapril exerts its pharmacologic effect by interrupting the renin–angiotensin system. Its oral bioavailability is sufficient to achieve systemic ACE inhibition, and its duration of action supports once- or twice-daily dosing depending on the clinical indication and patient response. The drug may modestly elevate bradykinin levels, a mechanism linked to both therapeutic effects and a potential dry cough in some individuals.
Enalapril is indicated for the management of essential hypertension, where reduction of systemic vascular resistance lowers blood pressure and reduces cardiovascular risk. It is also employed in symptomatic heart failure with reduced ejection fraction (HFrEF), where afterload reduction and neurohormonal modulation can improve functional status and clinical outcomes when used with standard therapy. The agent may be used following myocardial infarction in certain patients to improve remodeling and survival, as part of a comprehensive treatment plan.
Additional indications include management of nephroprotective effects in diabetic and non-diabetic nephropathies with proteinuria, where ACE inhibition can slow progression of renal impairment and reduce albuminuria. Enalapril is often used in combination with other antihypertensive or heart failure therapies as dictated by guideline-directed care and individualized risk assessment.
In all uses, therapy is tailored to baseline organ function, concomitant medications, and the patient’s hemodynamic status, with careful titration and monitoring during initiation and adjustment phases.
Starting doses for hypertension commonly begin at 5 mg once daily, with gradual titration based on blood pressure response, tolerability, and concomitant therapies. For heart failure, initiation is typically at a lower dose (for example, 2.5 mg daily) with slow upward titration as tolerated, aiming for an effective range often within 5–20 mg daily, adjusted to clinical response and renal function. The maximum daily dose for many adults is typically 40 mg, divided as needed, but individual plans may differ according to guideline recommendations and comorbidity.
Enalapril may be taken with or without food. Consistency in dosing time improves BP control and tolerability; patients should select a schedule that aligns with daily routines and remain adherent. In the setting of diuretic therapy or volume depletion, blood pressure and renal function should be monitored closely during initiation or dose adjustments. NSAIDs or other nephrotoxic agents can affect renal perfusion and counteract BP-lowering effects, so guidance from a clinician is advised when such medications are required.
If a dose is missed, take it as soon as remembered unless it is close to the time of the next dose. Do not double the dose to make up for a missed one. If multiple doses are missed or if vomiting occurs soon after dosing, contact a clinician for specific instructions. Storage should be at room temperature (loosely 20–25°C, 68–77°F), away from moisture and heat, in the original container with the cap tightly closed. Keep out of reach of children and pets, and do not use expired medication.
Common side effects include dizziness or lightheadedness, especially when standing up quickly, and a persistent dry cough. Fatigue, low energy, and headache are also reported. Some individuals may notice minor changes in kidney function or increases in blood potassium levels, which can be detected through laboratory tests. A small percentage experience a sensation of warmth or flushing after taking the medication.
Serious adverse events are less common but important. Angioedema, though rare, can present as swelling of the face, lips, tongue, or throat and requires immediate medical attention. Cough, facial swelling, or breathing difficulties should prompt medical evaluation. Kidney function and electrolyte balance should be monitored during therapy, especially in patients with preexisting renal impairment or those taking other agents that affect renal perfusion or potassium balance. Pregnancy is contraindicated in the second and third trimesters, and women should avoid becoming pregnant while using ACE inhibitors unless specifically guided by a clinician.
Consult a clinician promptly if you are pregnant, planning pregnancy, or breastfeeding, as ACE inhibitors can have serious effects on the fetus. Regular monitoring is advised, including blood pressure, kidney function tests, and electrolyte measurements, particularly potassium levels. Do not start or stop NSAIDs, potassium supplements, or potassium-sparing drugs without physician guidance, as interactions can affect safety and efficacy. Inform the prescriber of all medications, including over-the-counter agents and herbal products, and discuss any planned surgery or anesthesia, as perioperative management may require adjustments to therapy.
