Avanafil is a selective inhibitor of phosphodiesterase type 5 (PDE5) with a mechanism that enhances nitric oxide–mediated vasodilation in the corpus cavernosum, enabling erections in response to sexual stimulation. It is primarily used for the treatment of erectile dysfunction in adult men.
Avanafil belongs to the PDE5 inhibitor class and exerts its action by preserving cyclic GMP in penile smooth muscle after nitric oxide release. This pharmacologic effect facilitates penile blood inflow and contributes to the ability to achieve an erectile response with sexual stimulation.
Pharmacokinetically, avanafil is rapidly absorbed after oral dosing, with peak plasma concentrations typically reached within about 30 minutes. It is metabolized mainly by hepatic cytochrome P450 3A4 and has a half-life that supports as-needed dosing for erectile dysfunction.
Indication: Avanafil is indicated for the treatment of erectile dysfunction in adult men, to improve the ability to obtain and maintain an erection for satisfactory sexual activity. Efficacy depends on sexual arousal and stimulation.
Dosing and onset: The onset of effect is commonly observed within 15 to 30 minutes after dosing, with a duration that may extend for several hours. Avanafil is available in tablet strengths of 50 mg, 100 mg, and 200 mg, with dose adjustments based on efficacy and tolerability.
Contraindications: Avanafil is contraindicated in patients taking nitrates for chest pain or NO-donor therapies, due to the risk of profound and potentially life-threatening hypotension. It should not be coadministered with riociguat or in individuals with a known hypersensitivity to avanafil or excipients.
Precautions: Coadministration with alpha-adrenergic blockers may cause hypotension, especially at the start of therapy or after dose increases; careful monitoring and gradual titration are advised. Use with caution in patients with significant cardiovascular disease, severe hepatic impairment, or moderate to severe renal impairment. Strong CYP3A4 inhibitors markedly increase avanafil exposure; dose reduction or avoidance should be considered. Avoid excess alcohol, which can worsen hypotension or dizziness, and monitor for vision changes, hearing changes, or priapism, reporting promptly any concerning symptoms.
Common adverse events include headache, facial flushing, nasal congestion, dizziness, and back pain. These effects are typically mild to moderate and transient, often resolving with continued use or after dose adjustment.
Less frequent events include nausea, dyspepsia, diarrhea, abdominal pain, myalgia, or jaundice-like symptoms in rare cases. Rare but serious events may involve hypotension-related symptoms, syncope, severe allergic reactions, priapism, or, infrequently, sudden vision loss or hearing impairment; patients should seek urgent care if such symptoms occur.
Nitrates and NO-donor therapies: Avanafil is contraindicated with nitrates or NO-donor medications due to the risk of life-threatening hypotension. Coadministration with nitric oxide–modulating agents should be avoided.
CYP3A4 inhibitors, inducers, and other agents: Strong CYP3A4 inhibitors (for example, ketoconazole, itraconazole, ritonavir) raise avanafil plasma levels and may require dose reduction or avoidance. CYP3A4 inducers (such as rifampin, carbamazepine) decrease exposure and may reduce efficacy. Inform clinicians about all concomitant medicines, including over-the-counter and herbal products. Alcohol and other antihypertensives may enhance vasodilatory effects, potentially increasing the risk of dizziness or fainting. In all cases, adjust therapy based on efficacy and tolerability while monitoring blood pressure and pulse.
