Type 2 diabetes mellitus and obesity-related hyperglycemia are conditions characterized by persistent elevated blood glucose and excess body weight that contribute to cardiovascular risk and reduced quality of life. Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist indicated to improve glycemic control in adults with type 2 diabetes and, at higher doses, to support chronic weight management. The substance acts by enhancing glucose-dependent insulin secretion, slowing gastric emptying, and suppressing appetite, thereby reducing caloric intake and aiding weight loss.
Semaglutide is a synthetic peptide analogue of native GLP-1. It is a long-acting incretin mimetic that selectively activates the GLP-1 receptor to augment insulin release in a glucose-dependent manner and to reduce postprandial glucose excursions.
In type 2 diabetes, semaglutide improves glycemic control as monotherapy or in combination with other antidiabetic agents. For obesity or overweight, higher-dose regimens support weight loss when used alongside diet and exercise in adults with related comorbidities.
Contraindications include known hypersensitivity to semaglutide or any excipients; personal or family history of medullary thyroid carcinoma; and multiple endocrine neoplasia type 2. Pregnancy is not recommended during semaglutide therapy; breastfeeding should be avoided.
Precautions involve monitoring for pancreatitis and gallbladder disease, as well as for dehydration and renal function changes with reduced oral intake. Evaluate for signs of thyroid neoplasia in at-risk individuals; discontinue if suspicion of thyroid C-cell pathology arises. Use with caution in patients with significant gastrointestinal disease, including gastroparesis, and in those at risk of severe hypoglycemia when combined with insulin or sulfonylureas; dose adjustments may be required.
Very common (โฅ1/10): nausea, vomiting, diarrhea, decreased appetite, and constipation, along with injection-site reactions and headaches. Common (โฅ1/100 to <1/10): dyspepsia, abdominal pain, fatigue, dizziness, and back pain. Uncommon (โฅ1/1,000 to <1/100): pancreatitis, cholelithiasis, cholecystitis, dehydration, and hypoglycemia when used with insulin or sulfonylureas. Rare (<1/10,000): hypersensitivity reactions including angioedema; severe allergic reactions; rare cases of acute kidney injury. Unknown frequency: thyroid C-cell tumors in animals; human risk remains theoretical; monitor for thyroid symptoms and discontinue if neck mass, dysphagia, or persistent hoarseness occurs.
Concomitant insulin or sulfonylurea therapy increases the risk of hypoglycemia. Dose reductions may be necessary and glucose monitoring should be intensified during initiation and dose adjustments. GLP-1 receptor agonists delay gastric emptying, which can reduce the absorption of concomitant oral medications; administer such medicines at least 1 hour before or 11 hours after semaglutide dosing where feasible.
Oral contraceptives, antibiotics, and other small-molecule drugs may have altered absorption; assess timing and monitor therapeutic effects. If vaccines are planned, schedule injections to avoid interference with immune response. No clinically meaningful interactions with most antihypertensives or lipid-lowering agents have been identified; nonetheless, review all medications at initiation and adjust as needed.
