High blood pressure, or hypertension, can quietly damage your heart and kidneys. Heart failure can make everyday tasks feel harder and may cause shortness of breath. After a heart attack, your heart may need extra protection. Ramipril is a medicine that helps with these problems. It is an ACE inhibitor. Ramipril relaxes and widens your blood vessels so blood can flow more easily.

What Ramipril is

Ramipril is a prescription medicine taken by mouth. It comes as tablets or capsules. In your body, it becomes an active form that blocks a natural enzyme. This action relaxes the vessels and lowers the pressure inside them. Your heart does not have to work as hard.

Ramipril is an ACE inhibitor. It helps reduce extra strain on the heart and can improve symptoms in some people with heart failure or after a heart attack. It is often used alongside other treatments your doctor prescribes and is tailored to your situation and health goals.

What it is used for

Ramipril lowers high blood pressure by widening the blood vessels. This helps prevent damage to the heart, kidneys, and brain and can reduce the risk of stroke.

In heart failure, ramipril helps your heart pump more efficiently. You may notice less shortness of breath and better exercise tolerance as the heart works more easily.

After a heart attack, ramipril may lower the chance of further heart problems and help you recover. It can also be used to protect kidney function in people with diabetes or certain kidney conditions, where appropriate.

Your doctor will decide if ramipril is right for you and will consider any other conditions you have, such as kidney disease or diabetes. Dosing and duration depend on your situation and response to treatment.

Contraindications and precautions

Do not take ramipril if you are pregnant. Ramipril can harm your baby, especially in the second and third trimesters. If you think you might be pregnant, tell your doctor right away.

Do not take ramipril if you have had a severe allergic reaction to ramipril or other ACE inhibitors in the past. If you ever developed swelling of the face, lips, tongue, or throat while taking an ACE inhibitor, avoid ramipril and seek urgent medical care if swelling recurs.

Ramipril is not recommended for people with bilateral renal artery stenosis or a single kidney with severe impairment. It can worsen kidney function in these cases. Tell your doctor about any kidney problems you have.

Ramipril can cause very low blood pressure in some people, especially after the first dose or when your dose is increased. Signs include dizziness, lightheadedness, or fainting. Stand up slowly and sit or lie down if you feel unwell.

If you have high potassium levels or conditions that raise potassium, ramipril may worsen this. Your doctor may monitor your potassium and adjust your treatment. Rarely, very high potassium can cause heartbeat changes or weakness.

Angioedema, or swelling under the skin around the lips, tongue, throat, or face, can occur with ramipril. It may be life-threatening. It is more common in Black people and can happen at any time during treatment. Seek urgent care if you notice swelling, trouble breathing, or difficulty swallowing.

Dehydration from vomiting, diarrhea, or sweating can increase the risk of low blood pressure and kidney problems with ramipril. Tell your doctor if you have ongoing vomiting or diarrhea. You may need to drink fluids or adjust your dose.

Your doctor may want to check your kidney function, electrolyte levels, and blood pressure regularly while you take ramipril. If you have liver disease or a history of liver problems, your doctor may monitor you closely for signs of liver injury, though this is uncommon.

Side effects by frequency

Very common (more than 1 in 10 people): dizziness or lightheadedness, especially after the first dose or when the dose is increased. Cough is also common with this medicine and can persist for weeks to months.

Common (roughly 1 to 10 in 100 people): fatigue, headache, nausea, abdominal pain, and a mild rise in potassium or small changes in kidney function. Some people notice a skin rash or itching.

Uncommon (roughly 1 in 100 to 1 in 1,000 people): low blood pressure with symptoms such as fainting, rapid or irregular heartbeat, or sweating. Raised or decreased white blood cells can occur, along with chest tightness or wheezing in rare cases.

Rare (fewer than 1 in 1,000 people): angioedema (swelling of deeper tissues), liver enzyme elevations or liver injury, severe allergic reactions, severe kidney problems, and very high potassium levels. Some people may develop a persistent dry cough that interferes with daily life and requires medical review.

If you notice signs of serious side effects—swelling of the face or throat, trouble breathing, severe dizziness, fainting, yellowing of the skin or eyes, dark urine, or muscle weakness—seek medical help promptly. Always report any side effects to your clinician, especially if they are persistent or worry you.

Interactions with other substances

Talk with your doctor about all medicines you take, including over‑the‑counter drugs and supplements. Some medicines can interact with ramipril in ways that affect you.

Diuretics (water pills) can increase the blood pressure lowering effect of ramipril. Your doctor may adjust the dose and monitor you for dizziness or fainting when starting or changing treatment.

Nonsteroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen or naproxen can reduce ramipril’s effect on blood pressure and may increase the risk of kidney problems. Use NSAIDs only if your clinician says it is safe and monitor kidney function if you need them for a short period.

Potassium supplements, salt substitutes containing potassium, or certain potassium‑sparing diuretics (like spironolactone or eplerenone) can raise potassium to dangerous levels. Your doctor may check your potassium level and adjust your therapy accordingly.

Lithium, a medicine used for mood disorders, can interact with ramipril and affect lithium levels. Your clinician may monitor lithium levels and symptoms more closely if you take both.

Ramipril can interact with other blood pressure medicines. If you take several medicines for high blood pressure, your doctor will monitor your blood pressure and kidney function to keep you safe and comfortable.

In diabetes or kidney disease, a class of medicines called aliskiren can interact with ramipril and increase risks. Your clinician will decide if this combination is safe for you. Always inform your clinician of all health conditions you have and all medicines you take. Avoid starting or stopping therapies without medical advice.
